Community healthcare officially opens Blackthorn integrated care for older persons team

Minister for older people and mental health, Mary Butler, officially opened the DSKWW Community Healthcare Blackthorn Integrated Care for Older Persons (ICPOP) team on the Cherry Orchard Hospital campus in Ballyfermot.

The Blackthorn ICPOP unit is part of the national Enhanced Community Care programme rollout.

The facility has also welcomed a new Advanced Nurse Practitioner Féileacán Ban Nursing Service. This service provides multi element dementia supports through a structured service model of care and support directly aligned to Cherry Orchard Hospital

Speaking at the launch, Minister Butler said: “The teams and HSE staff I met with today are paving the way towards a bright future for older person quality care.”

The Blackthorn ICPOP team by its integrated design receives referrals from across the acute, community and voluntary sector. Self-referrals from patients known to the service are also accepted. Referrals can be sent from clinical and non-clinical persons.

This open approach allows older persons access specialist services as close to home as possible and can directly link them in with the most appropriate to their needs. After a referral is received the older person is visited by a member of the ICPOP team.
